Care built around the long game
Chronic conditions don't resolve in one appointment, so they shouldn't be treated like they will. We manage them over time — regular check-ins, medication adjustments as your body and life change, and monitoring between visits so problems get caught while they're still small.

Meds that keep up with you
What worked at diagnosis may not be what works now — doses drift out of date, side effects pile up, lives change. At each follow-up Latoya reviews how you're actually doing and adjusts medications accordingly. You'll always know what you're taking, why, and what we're watching for.
One provider who knows the history
With chronic disease, the history is half the diagnosis. When the same nurse practitioner follows your care visit after visit, nothing gets re-explained, nothing falls through the cracks, and decisions build on everything that came before. That's the relationship here — one provider who keeps the thread.
Common questions
I already have a specialist. Do I still need this?
Specialist care and primary care work together. We handle the day-to-day management — meds, monitoring, the in-between stuff — and coordinate with your specialist so everyone's working from the same page.
How often will I need to come in?
It depends on your condition and how stable things are — some patients check in every few months, others less often. Latoya will suggest a rhythm at your first visit and adjust it as things change.
Can you become my regular doctor for this?
Yes. Request a first visit at (267) 724-9300 and bring your records if you have them. Once the office confirms, we'll start building your plan together.
